One (1) ADA Federal Law Information Card (this card is only included for Service Dog Type options)
One (1) set of 2 Customized ID Badges (you will need to provide your dog's photo)

Very Important Note: You will need to get us a photo of your dog for your ID badges. You can either upload the photo or mail it to our physical location if that is easier.
